Croton Oil
The croton oil peel is my most favorite peel for deep vertical lines around the lips and mouth. It is a variant of the old phenol peel but allows one to control the depth of peel by varying the concentration of the croton oil. Since croton oil is a strong skin irritant, it provokes a marked inflammatory response during healing. The advantages I observe are a thickening of the skin (as opposed to thinning) with maintenance or a decrease in pore size (as opposed to an increase in pore size).
The primary disadvantage of the peel occurs during the first week when there is more swelling and inflammation in the skin with blistering and outer layer skin slough that looks bad. In the second week there is more redness than seen with laser treatment, but subsequently the healing is comparable and with the advantages previously mentioned. I typically use this peel as an adjunct to facelift. It enables the treatment of the vertical lines around the lips and mouth that are not affected by facelift. It is also very effective in the treatment of crepey eyelid skin that is not redundant enough for excision.
TCA
The trichloracetic acid (TCA) peel is a versatile peel that can be used in low concentrations (Blue Peel) as a step up from basic skin care. These peels are performed by our aestheticians in our skin care center (EpiCentre). Intermediate level peels are also possible with higher concentrations. I perform these peels either in the office or as a part of other surgical procedures. My best results have been in using TCA in the office as a touchup for recurrences of small lines after a croton oil peel.
