My Approach
There are several important decisions to be made with any breast lift cosmetic surgery in Dallas. As a plastic surgeon, I work closely with patients advising them of their options and determining what would work best for them. Careful clinical measurements of the chest and breast are an essential part of the evaluation. Women must consider a combination of incision and implant types.
Incisions:
- Periareolar - around the outside of the areola
- Periareolar with vertical limb - around the outside of the areola and a straight down incision (often called a "popsicle" incision)
- Inverted T - inframammary incision with a vertical incision from the middle of the inframammary fold to the aerola. This procedure is used when sagging is extreme.
Implants:
My patients choose either saline or silicone breast implants in Dallas for their augmentation mastopexy. I leave this choice to the patient. It is my opinion that silicone implants offer a superior result when combined with mastopexy. There is less recurrence of glandular and implant sag and better maintenance of the upper fullness of the breast.
Are You a Candidate?
There are many factors such as pregnancy, nursing, gravity, weight loss and even just the aging process, which may contribute to a significant loss in breast volume and skin firmness. As the skin loses its elasticity, the breasts lose their firmness and shape. If that describes you, and you're healthy and have reasonable expectations from the surgery, then you're a good candidate for a breast lift.
There are basically two types of candidates:
- Breast lift using implants - Look in a mirror. Imagine that you can see through the breast tissue to the chest and ribs. If the nipple is at or above the fold where the breast and chest meet, called the "inframammary fold" or "IMF", an implant may be used to fill out the top part of the breast and "lift" the breast to its original level.
- Breast lift by "mastopexy" - If while looking in the mirror you see that the nipple is below the "IMF," breast implants will not work unless a simultaneous mastopexy is performed. In cases such as this, the breasts are lifted surgically using suspension sutures in the deep structures of the breast and chest. Mastopexy repositions the nipple/areola complex and for this reason periareolar scars are always required.
How the Procedure is Performed
The Consultation
Requesting a mastopexy consultation in our Dallas plastic surgery office prior to surgery is crucial to a good surgical result. Measurements will be taken to pinpoint any asymmetries, and to determine if a breast lift and/or implants are needed to reach the desired outcome. The surgeon and patient should thoughtfully consider overall aesthetics when determining the ideal nipple position as well.
Be thorough in the selection of your Dallas plastic surgeon. Make sure he or she has thoroughly considered and explained the relation between breast lift, breast implants, and breast reduction in achieving your desired breast size and shape. One of the biggest mistakes in Dallas and elsewhere is to use over-sized implants in order to "lift" the breast when the nipples have fallen below the inframammary fold. That technique will not work and it's your responsibility to make sure that your surgeon knows that too.
Day of Surgery
Prior to sedation, measurements will be confirmed and the breasts will be marked to indicate the incision sites and implant position.
Surgery
Breast lift surgery carries little risk and can be done as an outpatient procedure in any accredited facility using IV sedation or general anesthesia. The majority of my cases are done at Dallas Day Surgery Center. The staff is exceptional and extremely skilled, and the facility is very comfortable for our patients and their families.
Incisions are made at the sites chosen by the patient and physician during consultation. The chosen size and style of implant is then placed either on top or underneath the pectoral muscle depending on patient desires and existing anatomy. Surgical drains are seldom required. A dressing is applied over the incision and a surgical bra worn for the first week. Routine medications include an antibiotic and an oral analgesic (pain medication).
