My Approach

My approach is not to use liposuction as a weight loss tool for Dallas plastic surgery patients. The attitude that obesity can be cured by liposuction is misleading and dangerous. My many years of experience with liposuction have clearly shown that the long term success of the procedure does not relate to the amount of fat that is removed, or the technique used to remove it, but rather to the patient's nutrition both before and after surgery. The success of this surgery lies as much in the hands of the patient as it does in the hands of the surgeon.

There are four major developments that influence the quality of result and overall satisfaction reported by our plastic surgery patients in Dallas / Fort Worth.

  • Small Canulas Used for Suction

    The small canulas, the equipment used to suction the fat deposits from the body, allow a more artistic approach in sculpting the superficial layers while producing much less trauma. I see a much smoother contour with the use of these small canulas.
  • Pre-Surgical Fluid Saturation of the Treatment Sites

    Saturating the sites prior to liposuction aides in the removal, minimizes blood loss, reduces pain and assists in doing ultrasonic techniques. This process is known as "tumescent infiltration."
  • Re-Use of Removed Fat for Fat Grafting

    Fat grafting by harvesting, purifying and then injecting the cells into depressions or wrinkles in the skin helps complete the sculptured, contoured appearance.
  • Ultrasonic Assisted Techniques

    Ultra-sonic techniques enable fat removal in more difficult zones.

Using all these technical advances along with keeping an artistic eye for the final aesthetic outcome is an approach that works well and to the best advantage for both patient and physician.

Perhaps due to the huge popularity of liposuction in Dallas and nationwide, there is another dangerous attitude growing in this field that any patient is a candidate. You may very well be a candidate, but again, liposuction should not be viewed as an alternative to weight reduction. Liposuction will not compensate for muscle and/or skin laxity. An ideal candidate is at or near their normal or preferred body weight, has good muscle tone with minimal skin laxity, and has localized, contained areas of fat accumulations that do not respond to diet and/or exercise. While almost any area of discrete localized fat accumulation can be addressed by liposuction, the sites that generate the best and most predictable results are:

  • Inner and outer thighs
  • Abdomen and the surrounding flank area
  • Waist
  • Inner knees
  • Inner arms
  • Chin and neck

How the Procedure is Performed

The Consultation

Liposuction, in Dallas and other metropolitan areas, in more recent years has been more descriptively referred to as "body contouring" or "lipo-sculpture." Liposuction is a technically and artistically demanding surgery. During the consultation the patient will make known their concerns and desired outcomes. It is common for a patient to focus on or be preoccupied with a single area. In an effort to make the body lines flow and for clothing to drape properly, it is sometimes necessary to "sculpt" surrounding body areas to provide a pleasing aesthetic line. Be open with your surgeon about what you want and remain open in listening to his recommendations for how to achieve your goals. If weight control has been an issue, we provide patients with diet and nutrition advice with special emphasis on carbohydrate intake (refer to the link on diet, nutrition and hormones).

The Day of Surgery

Prior to sedation, patient and surgeon will confirm the areas that are to be addressed. Contour marking will be done with the patient standing to enable accuracy and to compensate for the change of appearance when the patient is asleep and lying recumbent.

Surgery

Small incisions are made into which the canulas are inserted to begin the liposuction procedure. The canulas are moved across the area to be reduced until the most pleasing size and shape are achieved. If fat grafting is to be a part of the procedure, the fat is processed and readied for injection to any depressions or wrinkles in the skin.

Liposuction carries very little risk and can be done on an outpatient procedure in any accredited facility using IV sedation or general anesthesia. Most of my cases are done at Dallas Day Surgery Center. The staff is exceptional and extremely skilled and the facility is very comfortable for patients and their families.

Recovery: What to Expect

Days 1 - 7
  • A pressure garment will remain on until the third week post-op
  • Sponge baths only
  • Minimize activity for the first 2 days
  • Take pain medication as directed
  • Take antibiotics as directed
  • Drink plenty of fluids
Week 3
  • Pressure garment stopped at the end of the third week
  • Exercise can be started
  • Massage and/or ultrasound for maximum smoothness
Month 3
  • Routine check up and initial post-op photos
Annually
  • Routine check up
